MODIFICATION 2

CHANGES APPROVED NOVEMBER 2006

June 1, 2006 PRC Information System Change (83-C) - 0920-0650

To: Thelma Sims, Office of the Director, NCCDPHP, CDC

This package is to submit changes to the Prevention Research Centers (PRC) Program Information System (IS), OMB approval number 0920-0650. We are making these changes to our IS to incorporate our revised and specific national program indicators into the IS and to refine some sections of the IS from text boxes to discrete answers. These changes will give us the quantitative data set needed for our national program indicators and evaluation activities.

Please note that with these changes, the average respondent burden hours for the Information System do not change for our system.

Listed below are the deletions, modifications, and additions to reflect these adjustments along with a description of the function for each change. The specific questions being deleted from, modified, and added to the PRC IS are attached.
