In support of the request for an OMB clearance number under the Paperwork Reduction Act, the following two changes will be made to the previously submitted NSF Survey of Science and Engineering Research Facilities.

1. Page 1 of the survey will be modified to insert a new banner with the title “Confidentiality.” Under the banner the following paragraph will be submitted:

“Information provided on research animal space (Questions 1 row i, 3, 7, 8, 10, 12f, 15, 18, 21, and 24) and on the condition of S&E space (Question 6) will not be publicly available for individual institutions. In accordance with the National Science Foundation Act of 1950, as amended, and other applicable Federal laws, your responses will not be disclosed in identifiable form to anyone other than agency employees or authorized persons.”

2. For all questions cited in the above confidentiality pledge, the following line will be inserted above each question: “Reminder: Please see page 1 concerning the confidentiality of this item.”
