In accordance with TFM Vol. 1, Part 4
and DoD 7000.14-R, Vol. 5 there is a requirement that a payee
identify themselves and certify as to what happened to the original
check issued by the government: non-receipt, loss, destruction,
theft, etc. This collection will be used to identify rightful
reissuance of government checks to individuals or businesses
outside of DoD.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
