The Agency for Healthcare Research and Quality (AHRQ) hereby requests approval of a new information collection titled “Chartering Value Exchanges for Value-driven Health Care."

These data collection activities are not required by regulation and will not be used by AHRQ to regulate or sanction its customers. Participation by the public is entirely voluntary and confidentiality of respondents and their responses will be preserved. AHRQ is requesting that OMB expedite the review of this information collection.
