of Epi-X, particularly the emergency notification feature. This
data collection will evaluate the functionality and usability of
this feature.

questionnaire, and the order of possible choices could be
simplified without altering the burden to the recipient or the
number of people targeted for the survey. A revised survey has been
added to reflect content changes and, in some cases, the ordering
of the responses.
