Advance Notice of Shipment of Samples for HIV-1 Antibody Testing from the CDC Model Performance Evaluation Program


Purpose

The purpose of this letter is


  • to give you advance notice of the shipment of six plasma samples for HIV-1 antibody testing and


  • to check the accuracy of your shipping information.


The next shipment of samples for HIV-1 antibody testing will contain six plasma samples from individual donors who are HIV-1 infected or uninfected.


Sample Shipment date


January 23, 2007


Check information change form enclosed

On the information change form, please check the accuracy of the


  • shipping address,

  • mailing address,

  • contact e-mail address,

  • telephone and FAX numbers, and

  • current laboratory director's name.


No response is necessary if this information is correct.

Change required?

If changes are required, please make the appropriate changes, then


  • FAX the form to (404) 325-2667, or

  • call 1-800-642-6941, or

  • return the form directly to Constella Group, LLC in the enclosed pre addressed envelope.


Deadline for changes: December 22, 2006


Internal delivery

Some institutions require delivery of all incoming biologic specimens to

  • a mailroom or

  • a central receiving area in a different location at their address.


Note: Please arrange internally for timely delivery of the specimens to your laboratory.

If you do not receive your specimen shipment within 4 days after the shipping date or if the samples are damaged, please contact the


MPEP Coordinator, Zeptometrix at (508) 520-0588.
