Dear taxpayer,

Within the next few days, you will receive a request to complete a brief questionnaire about a letter you should have received from the IRS earlier this year. That letter was sent to taxpayers whose personal information had been used to file more than one tax return for the 2006 tax year. The letter advised those individuals that they may be victims of identity theft and told them of the steps they should consider taking to protect their identity.

In sending you this questionnaire, the Wage and Investment division of the Internal Revenue Service is trying to assess the effectiveness of its correspondence with the American taxpayer. As part of that effort, we are requesting your participation in this survey. Your opinions are important to us.

By asking about your thoughts on the letter, we hope to gather feedback from you that will help us identify the areas within the letter that work well and the areas that we need to improve.
