The Entity Compliance Calculation Report is an optional annual report and if used should be submitted in
addition to the DSF0700 report. Several compliance calculation items may be calculated on a corporate
basis; if compliance is demonstrated on a facility-by-facility basis, this form is not required. This form may
not be used to meet quarterly truck loading terminal reporting requirements.
Please do not leave any reporting fields in this report blank. When reporting, enter “NA” for fields that are
not applicable unless otherwise specified.
Annual reports using this form must be submitted by August 31st.
