Dear i-Say member,

We invite you to participate in a scientific research project funded by the National Institutes of Health. This research is focused on the health and well being of individuals with different neurological conditions. By neurological conditions we mean conditions that affect the nervous system such as Stroke, Epilepsy, Multiple Sclerosis, Parkinson’s disease, Amyotrophic Lateral Sclerosis or Muscular Dystrophy. However, you do not need to have a neurological condition to participate in this internet survey. We are also seeking responses from people in the general United States population who may not be diagnosed with a neurological condition.

This research is being sponsored by Dr. David Cella from Evanston Northwestern Healthcare (ENH) in Evanston, IL

The purpose of this study is to develop a questionnaire that helps to assess the quality of life of people with neurological conditions. This study will help develop a questionnaire that addresses the most important health-related aspects of people’s quality of life.

FREQUENTLY ASKED QUESTIONS

What more can you tell me about the purpose of this study?

Good measures of health can be used in many ways, such as monitoring the health of groups of individuals over time and providing information about how well services to individuals with neurological conditions are working. Our overall goal is to improve the measurement of symptoms and concerns of people with neurological conditions which can help healthcare researchers and providers address and improve quality of life.


Who are you interviewing for this survey?

We are seeking adults and children from the United States population to complete one survey, which will take approximately 30 minutes to complete. Your participation will be valuable in helping researchers to develop tools to better measure how neurological conditions and their treatments can affect individuals’ quality of life. The information that you provide also will help us understand how to interpret the results of the final health assessment tool

Remember, all the information you provide us is kept strictly confidential. It is used for classification purposes only. Your personally identifying information will never be given to anyone outside of Ipsos i-Say, and we absolutely promise never to try to sell you anything.
