APPLICATION FOR LICENSE OR MODIFICATION OF LICENSE FOR THE CONDUCT OF A PUBLIC WAREHOUSE

Warehouse Operator applicants use this form for new licensing or changes to existing licensing under the United States Warehouse Act (7 USC 241 et seq.)

Submit the original of the completed form in hard copy or facsimile to the Kansas City Commodity Office (KCCO), Warehouse License and Examination Division, STOP 9148, P.O. Box 419205, Kansas City, MO 64141-6205; or FAX 816-926-1774. Customers who have established electronic access credentials with KCCO may electronically transmit this form to KCCO. Features for transmitting the form electronically are available to those customers with access credentials only. If you would like to establish online access credentials with USDA, follow the instructions provided at the USDA eForms web site.

Warehouse Operator applicants not currently licensed under the USWA at the specified location in Item 1 complete Items 1, 2, 4, 5, 7A and 7B, 8, 11 and the signature block (Items 12A through 12F). Applicants currently licensed under the USWA for the specific location in Item 1 complete Items 1, 2, 3, 4, 5G, 6A through 6G, 7A and 7B, 8, 11 and the signature block (Items 12A through 12F).

Enter the City, County or Parish, and the State or Commonwealth of the principal location for which application is made for licensing. If applying for locations outside the principal state or commonwealth, list each principal location in each state or commonwealth.

Enter, if presently licensed at the location in Item 1, the current United States Warehouse Act license number, if known.

Enter the total estimated capacity of the warehouse (including, if an application for an amended license, the estimated capacity of the added or deleted facilities)

Enter a check mark in the "Original" box if making an application for a location listed in Item 1not currently licensed under the United States Warehouse Act. Enter a check mark in the "Amendment" box if making an application for a location currently licensed under the United States Warehouse Act.

Applicants

 

Those applicants who are not currently licensed (those not currently licensed warehouse operators who are applying for an original license) must file a current financial statement complying with the regulations cited and the Licensing Agreement. Timely consideration of this application requires completion and filing all of the required documentation.

By filing this application, the warehouse operator agrees to comply with the requirements of the Act, regulations, and applicable Licensing Agreement for Warehouse Operators.
