PRELIMINARY TITLE OPINION

The loan-closing agent completes this form. Rural Development (RD) provides this form to allow the loan-closing agent to advise the applicant and the local RD office of the results of their title examination of the property to be financed.

Submit the original of the completed form in hard copy or facsimile to the appropriate Rural Development servicing office.

Loan-closing agents must complete all of the items specified in the following table.

4 County	Enter the County of the property.

6 Period commencing with	Enter the date and time on which the examination began.
7 Filed for record on	Enter the date and time the examination was filed for record.
8 To	Enter the date and time the examination ended.
9 Sections III. through V	Complete information requested.
10 Date	Enter date form was signed.
11 Attorney�s Signature	Enter attorney�s signature.
12 Address	Enter attorney�s address.
