FINAL TITLE OPINION

The loan-closing agent completes this form. Rural Development (RD) provides this form to allow the loan-closing agent to advise the applicant and the local RD servicing office that the loan has been properly closed and is properly secured.

Submit the original of the completed form in hard copy to the appropriate RD servicing office. Customers who have established electronic access credentials with USDA may electronically transmit this form to the USDA servicing office. Features for transmitting the form electronically are available to those customers with access credentials only. If you would like to establish online access credentials with USDA, complete form AD-2016 or visit your local servicing office. AD-2016 is available from the USDA eForms service and must be notarized before mailing or faxing the form to USDA. It will not be accepted electronically.

 

Loan-closing agents must complete all of the items specified in the following table.
