LETTER OF INTENT TO MEET CONDITIONS

Applicants complete this form.  An applicant uses this form to advise Rural Development that the conditions in a letter of conditions for receiving a loan or grant are acceptable.

Submit the original of the completed form in hard copy or facsimile to the appropriate Rural Development servicing office.

Applicants must complete all of the items specified in the following table.

1 Date	Enter the date when the applicant�s representative signs this document.
2 Agency Name	Enter "Rural Development.".
3&4 Address	Enter the office address for the Rural Development office that issued the letter of conditions (street, city, state, zip).
5 Date	Enter the date (month, day, & year) from the letter of conditions.
6 Date	Enter the date (month, day, & year) when the organization expects to complete all items required by the letter of conditions.
7 Applicant Name	Enter the name of the applicant applying for the loan or grant.
8 Signature	The applicant's authorized representative signs this letter.
9 Title	Enter the title of the applicant's representative.
