This is a request for OMB to approve Revision of OMB No. 0925-0002 (Exp. 10/31/08) Individual Ruth L. Kirschstein National Research Service Award Applications and Related Forms.
The Public Health Service Grant Application PHS 416-1, Ruth L. Kirschstein National Research Service Award Individual Fellowship Application and the PHS 416-9, Individual Fellowship Progress Report for Continuation Support are used by applicants to request federal assistance for research-related training from the National Institutes of Health (NIH) and the Agency for Healthcare Research and Quality (AHRQ). The other related forms (PHS 416-5, 416-7, 6031, 6031-1) are used by these individuals to activate, terminate, and provide for payback of a National Research Service Award.
