Terms of Clearance: OMB expects that, prior to the next approval request for this ICR, the agency will develop electronic fillable forms for use at the Program/Facility level in an effort to reduce respondent burden. Also, as per conversations with SAMHSA, the agency must clarify in the form's definition section that sedation for non-medical purposes qualifies as restraint.

Short Statement: Currently, the total burden was 9,702 hours. CMHS is requesting 2,870 hours for the current project. The decrease of 6,832 hours is due to a programmatic change in the number of mental health facilities participating in the evaluation from 49 to 21 facilities.
