The burden hours for this
collection have increased from 250 hours to 1,608 hours. The
increase in burden represents a net change of 1,358 hours. In
addition, the number of responses has increased from 5,000 to
20,500. This represents a net change of 15,500 responses. The
increase is due to USDAs proposed use of the form (+1,458 hours
and +17,500 responses) and the Nuclear Regulatory Agencys
discontinued use (-100 hours and -2,000 responses).
