SUBJECT: Request for OMB Review and Clearance for the Collection of the Performance Report Form for the Paul Douglas Teacher Scholarship Program (CFDA 84.176)

Attached is the clearance package for the performance report form for the Paul Douglas Teacher Scholarship Program. The Program was authorized under Title V of the Higher Education Act of 1965, as amended. Both the Program statute (Title V, Part C, Subpart 1) and regulations (34 CFR 653) have long since been repealed. The Program was last funded in 1995. The expiration date for the attached form is February 28, 2009.

The attached OMB Form 83-I and supporting documents are being submitted as required for approval of a data collection instrument. The Department uses the information submitted on the Paul Douglas Teacher Scholarship Program Performance Report to ensure the compliance of the State Educational Agencies and the level of fulfillment of the scholarship obligations by the Douglas scholars.
