SUBJECT: Health Resources and Services Administration Uniform Data System (UDS)— OMB No. 0915-0193

This is a request for a non-material or nonsubstantive change to a currently approved OMB approval for the Health Resources and Services Administration’s Uniform Data System (UDS), the annual reporting requirement for health centers funded under Section 330 of the Public Health Service (PHS) Act. The Health Resources and Services Administration (HRSA), has responsibility for the administration of the health center programs under Section 330. An extension of the UDS was recently approved under OMB No. 0915-0193 and expires on 01/31/2011.

The attached material is provided in support of a request for approval for a non-material/nonsubstantive change to the Uniform Data System. The change consists of how race and ethnicity are collected at the health center level. The new table is provided for your review along with an updated burden table.

This is the 12th submission under that approval and will impose an estimated annual average total burden of 1,076 hours, which would bring the total currently approved burden under this clearance to 67,988 hours for this annual data collection. The current total burden hours in the OMB inventory for the Uniform Data System (OMB No. 0915-0193) is 66,912.
