NSCCSD Design Phase Feasibility Test

Before/After School Programs Screener – Revised 12/17/08

Hello, I am calling from the National Opinion Research Center at the University of Chicago. We are conducting a study to determine the types of programs serving children under age 13. I’m calling today to learn if your program provides certain types of programming for that age group. May I take fewer than 5 minutes to learn a bit about your program?

1. First, let me confirm that I have called {PROGRAM} located at {ADDRESS}. Is that program name correct?

1 Yes

2 No (Specify: _________________)

1a. And is that the address where children actually participate in programming through your organization?

1 Yes

2 No (Specify: ____________________)

1b. Do you operate your program in a single site, or do you operate a multi-site program? IF R’S PROGRAM IS ONE SITE IN A MULTIPLE SITE OPERATION, CODE SINGLE SITE. IF R’S PROGRAM IS MULTI-SITE, RECORD NAMES OF SITES.

1 Single site

2 Multi-site (Specify _______________________________________________)

2. Do you currently serve children under age 13 in before or after-school programming? IF A PROGRAM EXISTS BUT NO CHILDREN ARE CURRENTLY BEING SERVED, CODE NO.

1 Yes

2 No (GO TO Q8.)

3. Do you offer programming to children under age 13 at least 3 days per week and at least 2 hours per day?

1 Yes

2 No

4. Does your program offer a single activity, or does programming entail more than one activity? For example, an after-school basketball program or a tutoring program would probably be single activity programs.

1 Single activity

2 Multiple activity

5. Are parents or guardians expected to be present while children under age 13 participate in your programming?

1 Yes

2 No

6. Can children participate in your programming on a drop-in basis, or must they be enrolled in advance?

1 Drop-in permitted

2 Advanced enrollment required

7. Is your program limited to school holidays or vacations?

1 Yes

2 No

8. Those are all of my questions today, thanks very much for your time.
