	(Stocks Only Seasonal)	Dear Potato Grower: The Department of Agriculture will estimate stocks of potatoes on farms and in hands of dealers on December 1, January 1, February 1, March 1, April 1, May 1, and June 1 in the States producing fall potatoes. Response to this survey is voluntary and not required by law. However, your help is needed to measure these stocks during the season. Each time you receive this form, please report in the appropriate column and return it to us promptly. Your report will be confidential and used only for arriving at State and National figures. The enclosed envelope for returning the questionnaire does not require a stamp. Your cooperation is appreciated.
Please make corrections to name, address and Zip Code, if necessary.

INSTRUCTIONS

• Report for the farm(s) you operate in this State.

• Report potatoes stored on this farm, on nearby farms and in off-farm storages.

• Include potatoes sold but not delivered.

• Include quantities sold and to be sold for fresh market, seed, processing, and livestock feed.

• Include quantities to be used for food, feed and seed on farms where grown, and quantities purchased for seed.
