This information collection covers the collection of information from Hazardous Liquid operators. This request is being submitted to OMB to address only revisions to the Hazardous Liquid Accident Forms for Hazardous Liquid operators. To ensure adequate public protection from exposure to potential natural gas and hazardous liquid pipeline failures, PHMSA collects information on reportable natural gas pipeline incidents and hazardous liquid pipeline accidents. Additional information is also obtained concerning the characteristics of an operator’s pipeline system.

PHMSA is revising the Accident forms for Hazardous Liquid Operators to enhance incident data provided by such operators in response to recommendations from the National Transportation Safety Board (NTSB) and the Government Accountability Office (GAO). In addition this Information Collection is being revised to remove certain hours for Annual Reporting that are now accounted for in another Information Collection.
