Re: 2009 Cable Industry Price Survey

Dear Cable Operator:

The Media Bureau of the Federal Communications Commission is conducting its annual survey of cable industry prices, as required by the 1992 Cable Act. Pursuant to the enclosed Order, your response to this price survey is mandatory. Please provide the information requested for each community identified on the attached list. If this list contains more than one community unit identification (CUID) number, complete an individual questionnaire for each of these CUIDs.

This year, the survey questionnaire is entirely Internet-based, which means that in addition to being able to access the survey questionnaire from the Commission’s Internet site (which you could do in prior years), you must now enter your answers to the questions directly into the questionnaire and file your response via the Internet. Paper filings will no longer be accepted. If you do not have access to the Internet, please contact my staff by phone at (202) 418-2330. This new format will expedite your completion and our processing of your survey submissions.

As in prior years, you can access the questionnaire at the Commission’s Internet site, at http://www.fcc.gov/mb/2009. Once you have accessed the questionnaire, you will need to complete a login process before you can enter the questionnaire itself. The login process includes an FCC Registration Number (FRN) and a password. Once you have completed the login process, you can proceed to enter the questionnaire. There is no longer any need to download the questionnaire into a Microsoft Excel workbook. When you are ready to submit your completed questionnaire to the Commission, you click the “Submit” link at the end of the questionnaire and your completed survey will be entered automatically into the appropriate data base at the Commission.

Your response to this price survey must be filed with the Commission by June 8, 2009. General instructions for completing the survey are set forth in Appendix B of the accompanying Order. A more detailed set of instructions for dealing with the web-based survey can be found under the link for “Instructions” at the Internet site identified above.
