Dear Reporter:
Your help is needed to prepare estimates of vegetable acreage for fresh harvest during the period of January 1 - March 31, 2009. Information for your individual firm will be kept CONFIDENTIAL. Response to this survey is voluntary and not required by law.
Please complete and return this questionnaire by December 10. A postage-paid envelope is enclosed for your convenience or you may fax it to 1‑888‑478‑5637. If you have any questions, please call Sarah DeVandry at 1-800-851-1127, Ext. 143. |
INSTRUCTIONS
1. Please report all California acreage from which this firm EXPECTS to ship during the specified period. Also, report last year's FINAL acreage for the same period. This will allow us to make any corrections to last year's reported acreage and compare both years in percentage terms. Please estimate, if necessary, for both years to reflect changes in acreage for the crops listed below. Include acreage you will ship from other growers. If your firm ships from more than one location in California, please include acreage from all points.
2. For the crops listed, if you WILL SHIP NONE THIS YEAR and DID NOT SHIP ANY LAST YEAR during the JANUARY-MARCH PERIOD, please check this box and return this survey.
