Instructions for "ROUNDWOOD RECEIVED FOR PROCESSING"
This instruction sheet is provided to help you fill out the "ROUNDWOOD RECEIVED FOR PROCESSING" survey form. This form has been designed to determine the location, size, and composition of the primary wood-using mills in your state, the volume of roundwood harvested by product, species, and geographic location, and the volume and disposition of wood residues generated during primary processing for the calendar year indicated. Your cooperation in completing this form will make the results of this survey comprehensive, accurate, and timely.
Please complete this form and return it promptly. If complete records are not available, please give your best estimates. All information is held in confidentiality and used only for statistical reports. Information may be used to compile "Regional/Statewide Industry" directories and maps, BUT NO individual mill production data will be released. Your cooperation is appreciated.
Page 1 -- Mill Information
* Fill in the company name and mailing address, and the name and phone number of the owner, manager, or person that can be contacted if there are any questions concerning the information on the survey form.
* Record the name of the County in which the mill is located and the city or town the mill is nearest to. Note: If Latitude and Longitude of mill location are available, they may be entered instead of city or town.
* Check the box under "Type of Roundwood Processed" that indicates the type of roundwood received at the mill for processing. Complete a separate form for each “Type of Roundwood Processed”.
* Indicate the amount of wood the mill is capable of processing in a year when operating at full capacity. Please indicate unit of measure if other than thousand board feet per year.
* Check the box under "Mill Type" that best describes the mill. If none of the types listed describe the mill, write in the type of mill after "99-Other: (Specify) __________".
* Under "Primary Product Produced", list the 3 main products produced for the type of roundwood processed.
Page 1 -- Mill Receipts
* Check if you received roundwood in year. If not, no further information is needed
* If 'YES' to receiving roundwood in year, enter the volume that was received for the type of roundwood processed and check the unit of measure.
* Indicate the percent of the volume received that came from the county the mill is located in, the rest of the State, and other States that roundwood was received.
* Indicate the normal one-way procurement radius of roundwood received at the mill.
Pages 2 -- Mill Receipts
* Enter the percent of the volume received, or the actual volume received, for each species. Total of percents should equal 100% or total for volume should equal total volume received.
Pages 2 -- DISPOSAL OF BARK AND WOOD RESIDUES GENERATED
* Indicate how the wood and bark residues generated at the mill are disposed of by placing percentages in the boxes under the appropriate wood-residue type columns. The percentages in each of the residue type columns should sum to 100% for the species groups (softwood/hardwood) used by the mill.
Pages 2 -- EXPORT LOGS
* Indicate if any logs are resold outside the United States. A separate form should be filled out with Mill Type of "24 - Export logs" checked on page 1. Only record the volume of logs exported out of the country on the Export logs form. DISPOSAL OF BARK AND WOOD RESIDUES GENERATED does not need to be completed for any logs that are shipped out of the Country.
Pages 2 -- NOTES
* Use the 'NOTES' section to record any information about the mill that may be helpful to distribute volumes to counties of harvest for roundwood received at the mill. Some examples: 1) If all of a species comes from a specific area (county, group of counties, or specific State). 2) Indicate if most of the roundwood received at the mills is harvested in a certain direction, indicate volume or percent and direction.
