We made a few slight revisions to the information collection #1820-0677 form after the 60-day comment period. The revisions include:
Inserting the phrase “through all dispute resolution processes” to the instructions for reporting mediation requests (row 2) (on page 2 of the instructions and table grid);
Inserting the text “withdrawn or” into the phrase “mediations not held (row 2.3) (on page 3 of the instructions and table grid”; and
Inserting the text “(including resolved without a hearing)” to the instructions for reporting due process complaints withdrawn or dismissed (row 3.4) (on page 4 of instructions and table grid).
For this reason, we submitted the form as a revised information collection. Thank you
