Abstract: This package provides instructions and forms necessary for States to report the number of written, signed complaints; mediation requests; due process complaints; and expedited due process complaints and the status of these actions with regards to children served under Part B of IDEA initiated during the reporting year. The form satisfies reporting requirements and is used by OSEP to monitor SEAs and for Congressional reporting.
