Abstract: This package provides instructions and forms necessary for States to report the number of of children with disabilities who are receiving special education and related services under IDEA, Part B, but were removed from their special education placements due to disciplinary offenses. The form satisfies reporting requirements and is used by OSEP to monitor SEAs and for Congressional reporting.
