The U.S. Department of Agriculture (USDA), Center for Nutrition Policy and Promotion, in support of the 2010 Dietary Guidelines for Americans (DGA), has developed recommendations for making choices that add up to healthy and balanced eating habits. The USDA is sponsoring a study to collect information, which will guide the development of communication efforts to support the DGA. The USDA is interested in the opinions of children ages 10-13, and is requesting your consent to invite your child to participate in the survey.

Your child's participation in this survey is voluntary. There will be no penalties if your child decides not to respond, either to the information collection as a whole or to any particular question. Furthermore, the information your child provides will remain confidential and will not be disclosed in identifiable form to anyone.
