Terms of Clearance: This collection is cleared for 6 months with the following terms of clearance. DOD needs to re-submit this collection at the 30 notice stage before July 2010. The revised package and form should contain the following. 1. The purpose of the form -- What is the authority to collection this information? What is DOD’s authority to ask for SSNs in this case. 2. The process the applicant will go through. 3. A discussion of the disclosure of the information – DOD needs to describe who this information is going to be shared with and the privacy act routine uses 4. An authorization for release of the information -- this needs to include a signature -- as well as how long the authorization is valid (probably not more than a year).
