We are collecting information on chemical applications for floriculture and nursery production in 2009 and need your help to collect
complete and accurate data. Authority for collection of information on the Nursery and Floriculture Chemical Use Survey is
Title 7, Section 2204 of the U.S. Code. This information will be used to compile and publish estimates on chemical use in floriculture and
nursery production. Response to this survey is confidential and voluntary.
