CSAT User Registration Instrument

Chemical Security Assessment Tool User Registration

Scope of Instrument

This instrument collects:

• Personally identifiable information of the individual applying for a CSAT User Account

• Whether or not the individual applying for a CSAT User Account is a US Citizen

• Whether or not the individual applying for a CSAT User Account is domiciled in the US

• Whether or not the individual applying for a CSAT User Account is an Officer of the corporation

• Whether or not the individual applying for a CSAT User Account has been designated by an Officer of the Corporation

• Demographic information about the facility to which the individual is being assigned to provide information about

• Contact and location information about the facility to which the individual is being assigned to provide information about

• Updates and changes to personally identifiable information as well as the transfer of responsibilities between CSAT Users

• Updates and changes to facility demographic, contact and location information

Attached are DHS Forms 9002, 9002.1, 9002.2, 9002.3, and 9002.4 that standardize the collection of CSAT User Registration under this instrument. DHS Forms 9002.X identifies most, but not all of the data routinely collected under this instrument.
