Dear Dr. McKeown:
The University of South Carolina Institutional Review Board (IRB) approved Amendment
Ame1_Pro00004020 by Expedited review on 5/5/2010.
Brief description of changes:
Data collection for Wave 4 and 5 constitutes a program change due to the revision of the data collection
procedures and instruments for the youth participants who have reached high school age. This
amendment includes new and modified data collection instruments and a modified incentive structure.
A new consent form for 18 year old and older participants has been added and previous consent
document for the parent consent for 9-17 year olds and the child assent form for 9-17 year olds have
been modified.
Note: Approved consent document(s), if applicable, are located under the “Stamped ICF” tab on the
Study Workspace screen in eIRB.
