Dear «Salutation»:

The Federal Communications Commission (FCC), an independent agency of the United States government, would like your help in completing a national survey of businesses. The FCC wants to hear from decision makers about their business’ use of technology and the internet and has asked PSRAI to conduct the survey.

It is important that your opinions are included to ensure that the survey accurately represents the views of businesses -- small, medium and large in size. The data from the survey will be used by the FCC to develop the National Broadband Plan mandated by federal law. The plan will focus on how to improve access to the internet for all Americans, with a unique focus on U.S. businesses.

The survey can be completed either online or by telephone, and should take no more than about 20 minutes of your time. To complete the survey online, please go to the following survey web address and enter the user name below to log in:

http://xxx.xxx.com Type in this user name: «USERNAME»

If you choose not to complete the survey online, an interviewer from Braun Research Inc. will call you in a few days to arrange a convenient date and time for an interview. If you prefer, you can call this toll-free number 1-xxx-xxx-xxxx to schedule an appointment at any time that is convenient for you. Your answers will be kept private by the research firms. No information that can identify you or your company will be provided to the FCC or any other third-party.

Thank you in advance for your participation in this effort. If you have any questions concerning this study, please contact [email protected].

Sincerely,

G. Evans Witt
