NRC Form 664 is used by NRC general licensees to make reports regarding certain generally licensed devices subject to annual registration. The registration program allows NRC to better track general licensees, so that they can be contacted or inspected as necessary, and to make sure that generally licensed devices can be identified even if lost or damaged. Also, the registration program ensures that general licensees are aware of and understand the requirements for the possession, use and disposal of devices containing byproduct material. Greater awareness helps to ensure that general licensees will comply with the regulatory requirements for proper handling and disposal of generally licensed devices and would reduce the potential for incidents that could result in unnecessary radiation exposure to the public and contamination of property.

The change in the overall burden estimate for licensees to register general licensed devices containing radioisotopes on NRC Form 664 has decreased because of a re-estimate of the number of annual responses based on the actual number of responses received during the past 3 years. The number of registration responses are expected to decrease by 160 (from 1,000 to 840) resulting in a reduction of 53 hours (from 333 hours to 280). The rate has increased from $214 per hour to $257 per hour in accordance with Part 170. The change in annual burden overall estimate for registration of NRC authorized general licensed devices (GLDs) containing radionuclides on NRC Form 664 has increased by $698 (from $71,262 to $71,960) primarily due to an increase in the hourly rate from $214 to $257 and due to a revised number of expected enquiries resulting from the inclusion of GLDs containing Nuclear Accelerated Produced Materials (NARM) for the first time.
