OUTLINE FOR COLLECTING SCHOOL RECORDS, PERFORMANCE, AND PROGRAM PARTICIPATION DATA



The study team will take the following steps to collect school records, performance, and program participation data for the Evaluation of Adolescent Pregnancy Prevention Approaches (PPA).



  1. The study team will work with the school districts or other organizations to identify contacts. Districts may choose someone at the district level as their contact or, because some data may be decentralized, they may choose to designate school-level contacts; the same variance may occur with organizations.


  1. The team will send a packet of materials to each contact that includes the following:

  1. A letter explaining the purpose of the school records, performance, and/or participation collection and detailing the information to be collected. The letter will state that the records can be provided electronically or as a hardcopy printout, whichever the contact prefers.

  2. A list of the students for whom we are requesting information.

  3. A list of the items being requested (the list of items may differ depending on the site, e.g. most organizations will not collect data on some of the items below):

    • Program participation information

    • District identification number

    • Date of birth

    • Gender

    • Race

    • Ethnicity

    • Eligibility for free/reduced-price lunch

    • Whether the student is an English language learner or limited-English proficient

    • Whether the student receives an individualized education plan

    • Grade point average

    • Current grade level

    • Grade promotion

    • Standardized test scores

    • School Attendance

d. Instructions for submitting electronic data and/or returning a hardcopy printout.


  1. The study team will conduct calls with each contact to answer questions and follow up with non-respondents.
