Justification Statement

EPA is submitting this non-material change request to request OMB approval of the following forms.

• Qualified Product Information Form for Set-top Boxes (EPA Form Number 5900-206)

• Qualified Product Information Form for Computer Servers (EPA Form Number 5900-207)

• Qualified Product Information Form for Commercial Griddles (EPA Form Number 5900-208)

• Qualified Product Information Form for Commercial Ovens (EPA Form Number 5900-209)

• Qualified Product Information Form for Audio/Video Products (EPA Form Number 5900-210)

• Revised: Qualified Product Information Form for Commercial Refrigerators and Freezers (EPA Form Number 5900-68)

As noted in the supporting documentation for the ENERGY STAR Labeled Products ICR, each year a limited number of additional products are expected to be added to the program. These products are consistent with the general product categories listed in the supporting statement for the last renewal. The type of information requested in these form is similar to the information request in the already approved ENERGY STAR forms.
