Telephone Follow-Up with Non-Respondents



INTRO: Hi, my name is ____ and I’m calling from ICF International on behalf of the National Institute for Occupational Safety and Health, or NIOSH. May I please speak with [name of Mine Operator or Mine Safety Manager]?

[If the Safety Manager or Mine Operator is not available, phone representative asks for direct line phone number to call back]


RESPONDENT on phone Good afternoon. We’re doing a follow-up on behalf of the NIOSH Mining Program to make sure that you have received the Survey of Mine Interventions. This is not a sales call. According to our records we mailed a copy of the survey to you at the [Mine Name] on [date]. Do you recall whether you received it?


IF NO: I see. Well, to be brief, [Mine Name] is one of 300 mines selected to participate in a survey to assess whether NIOSH’s safety interventions meet the needs of the coal mining industry. NIOSH is using the findings to enhance the awareness, use and effectiveness of its safety innovations. The survey—which also is available to complete over the Internet—is completely voluntary, and will only take about 30 minutes to complete. We’d like to send you a new copy; is the correct address for [Mine Name] still [Give address]?


Thanks—you should see it in the mail in 2-3 days. You can fill out the paper questionnaire and return it in the self-addressed, stamped envelope, or you can go online to complete the survey, whichever is more convenient for you. We’ll send you a unique ID and password along with the paper copy. [Go to WRAP-UP]


IF YES: Oh, good, you did receive it. Because we hope to begin tabulating the data and preparing survey findings in the next 2-3 weeks, we could really use your mine’s input on these questions about mine safety. We would like to know if you would be willing to complete the survey within the next day or two and send it back to ICF International, the survey contractor. Can we count on you to complete your survey, either using the printed questionnaire or online?


IF NO: Well, we thank you for your time.


I F YES: Great! The website address and your mine’s unique password and ID were provided in the package? Do you need me to give it to you again? Or can I provide you with the return address for the survey again? [Provide either or both at respondent’s request]
