pdfCrosswalk for CMS 906 OMB # 0938‐0469
Explanatory Paragraph:
The reason for modification to this collection effort is to add financial information collected under the Medicare Part D Reporting
Requirements Section XIV (OMB # 0938‐0992) to the Fiscal Soundness Reporting Requirements collection (OMB # 0938‐0469). The
Fiscal Soundness Reporting Requirements document (Fiscal Soundness Reporting Requirements.doc) in this collection effort actually
originates from the Part D collection because there was no document that specified what was to be reported by the organizations under
the Fiscal Soundess Reporting Requirements collection. Once this collection effort is approved Part D (MDBG) will submit a request to
remove the Licensure & Solvency, Business Transcactions and Financial Reporting Requirements section from the Medicare
Part D Reporting Requirements. Both the original document referred to in this paragraph and the Fiscal Soundness Reporting
Requirements document in this collection effort are instructional.

Title change reflects Fiscal Soundess Reporting Requirements instead of Financial and solvency requirements.
Adds PDPs data elements requirements to this collection (increase). Change was also made to original document to
include data elements collection for MAOs, Demonstration plans and 1876 Cost Plans. The data elements for those
organization types were already being collected under OMB # 0938‐0469 thus there is no increase in burden. Data
elements being collected from all organization types decrease to four but this decrease is neglible. The addition of
Part D in this section increase the overall burden in this section.
