2012 Survey of Business Owners – Cognitive Interview Protocol

Before the Questionnaire:

  • What kind of business do you have? What types of products or services do you provide to your customers or clients?

  • Do you recall receiving this form in 2007? If so, what do you remember about it? Do you recall whether you completed it and returned it to the Census Bureau?


Now let’s take a look at the form.

  • (If R received and completed the form in advance) I’d like to go through and ask you about your answers.

  • (If R is seeing it for the first time) I’d like you to approach the form as if you had received it in the mail, and go ahead and fill it out. I’ll ask you questions as you go through the form.
