Dear [STUDY PARTICIPANT]:

Thank you for your continued participation in the National Children’s Study (NCS).

As part of your participation in the NCS, we would like you or someone else in your home to collect some environmental samples. These samples are similar to the samples the NCS Technician collected from your home at an earlier visit. It is safe for you to take these samples yourself while you are pregnant, however, if you do not want to collect them you may ask a friend or family member to collect the samples for you.

In this package, you should have everything you need to collect these environmental samples. This package should contain:

• 1 – “One Air Badge Collection Kit”

• 1 – “One Air Badge Instructions and Questionnaire”

• 1 – “Dust Plate Collection Kit”

• 1 – “Dust Plate Instructions and Questionnaire”

If you have any questions or concerns regarding the collection of these samples, please call the NCS at 800-XXX-XXXX.
