Appendix A A.2.3.u–1

Dear Study Participant:

The National Children’s Study asks many questions about how children’s environments might affect their health and well-being. Part of the study is to look at the quality of drinking and bathing water in participant’s homes, which is why we are collecting a water sample from your tap.

To learn more about the safety of well water, we have enclosed the EPA report, “Drinking Water from Household Wells”.

For more information about maintaining the quality of your drinking water, you can contact the state and local officials in your area listed below:

LIST DEPARTMENTS RESPONSIBLE FOR WELL WATER QUALITY IN STUDY LOCATION.

You can also contact the National Safe Drinking Water Hotline at 1-800-426-4791.

Sincerely,

Local Investigator
