for the survey.
According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a
person is not required to respond to, a collection of information unless it displays a valid OMB

Research and we’re conducting a survey for the U.S. Department of
Agriculture.
Taking part is voluntary and all the information you give us will be kept

the foods you get during the week we will mail you a $25 telephone
bonus.
A one‐person household may get $75 if he/she completes all survey
activities; a four‐person household may get up to $135 for completing
all survey activities.
