FOR BORROWERS WHO RECEIVED FORM FSA 2512

This form is used by borrowers, who are less than 90 days past due, to request loan servicing on all their direct Farm Loan Program loans. It is submitted in response to the receipt of a Form FSA 2512 and a servicing application package.
