Abbreviations: “SC” = sampled child. “HCP” = health care provider. “CSHCN” = children with special health care needs. “NSCH” = National Survey of Children’s Health.
Deletion:
How well respondent speaks English (asked only of respondents for whom the NSCH interview was not conducted in English)
Section 2: Health and Functional Status
