2011 National Survey of Children’s Health


Summary of Additions, Deletions, and Revisions, by Section

January 21, 2011


Abbreviations: “SC” = sampled child. “HCP” = health care provider. “CSHCN” = children with special health care needs. “NSCH” = National Survey of Children’s Health.


Section 1: Initial Demographics


Deletion:

  • How well respondent speaks English (asked only of respondents for whom the NSCH interview was not conducted in English)


Section 2: Health and Functional Status


Additions:

  • Age at first diagnosis

    • Attention Deficit Disorder (ADD), Attention Deficit Hyperactivity Disorder (ADHD)

    • Anxiety problems

    • Behavioral or conduct problems

    • Autism, Autism Spectrum Disorder (ASD)

    • Developmental delay

    • Intellectual disability

    • Cerebral palsy

    • Speech or other language problems

    • Tourette Syndrome

    • Vision problems that cannot be corrected with standard glasses or contact lenses

  • Follow-up questions for children ever diagnosed with autism or ASD

    • Type of doctor or other HCP who first told you SC had autism or ASD

    • To the best of your knowledge, did SC ever have autism or ASD

    • Reasons why SC may no longer have autism or ASD

      • Treatment helped condition go away

      • Condition went away on its own

      • Behaviors or symptoms changed

      • A doctor or HCP changed the diagnosis

      • Any other reasons (verbatim)

    • Reasons why a doctor, HCP, or school professional may have told you SC had a condition (he/she) never had

Deletions:

  • Follow-up: Are SC’s current problems related to his/her bones, joints, or muscles

Revisions:

  • Dental health

    • Past 6 months did SC have toothache

    • Past 6 months did SC have decayed teeth or cavities

    • Combined these two separate questions into one question, which was moved to Section 4, Subdomain 2

    • Added “unfilled” before “cavities” in the new question

    • Lengthened reference period from “past 6 months” to “past 12 months”



Section 3: Health Insurance Coverage


Deletions:

  • Difficulties enrolling in health plans

    • Past 12 months –tried to enroll SC in new health care plan

    • Ease or difficulty to complete application

    • If difficult, why

  • Burden of expenses

    • Respondent or other family members pay all or part of the cost of family’s health insurance

    • Cost of health insurance cause financial problems for family


Additions:

  • Expenses and barriers to care

    • Cost of SC’s health care caused financial problems for family (wording aligned with the National Health Interview Survey question; also slightly adapted for the NSCH)

    • Frustrated in efforts to obtain health care services for SC



Section 4: Health Care Access and Utilization


Additions:

  • Home visitation (for SC less than 4 years of age only)

    • Any home visits from someone from a maternal, infant, and early childhood home visiting program

    • If so, number of professionals that visited

    • If so, concerns about children and families that may have been discussed

  • Vision screening

    • Did SC receive vision testing with pictures, shapes, or letters (ever < 5 years old SC, past 2 years > 5 years old)

    • Kind of place/places where SC received vision test(s)


Deletion:

  • Child needed care right away from his/her personal doctor or nurse for illness or injury



Section 5: Medical Home


Deletions:

  • Needed an interpreter to speak with SC’s doctors or other HCP

  • When needed an interpreter – how often was respondent able to get someone other than a family member to translate


Section 6: Early Childhood (0 – 5 years)


Additions:

  • Flourishing

    • (attachment/bonding): affectionate & tender with you

    • (happy/joy): smiles & laughs a lot


Revisions:

  • Screen & media time on an average weekday (split into two separate questions)


Deletions:

  • Receipt of specific information to address concerns from SC’s doctors or other HCP (if the respondent had any concerns about SC’s learning, development, or behavior)

  • Child care

    • Among children who received at least 10 hours of care in non-relative setting, where child care was provided

    • Did child ever receive at least 10 hours of care per week from a relative other than parents/guardians

    • Where this care from a relative was received

    • Past month, did respondent need child care for SC

    • Did SC’s behavior limit respondent’s ability to find child care for SC

    • Did SC’s health limit respondent’s ability to find child care for SC

    • During past month how many times did respondent need to make different arrangements for child care at the last minute due to circumstances beyond his/her control

  • Injury

    • Past 12 months/since birth – has SC been injured & required medical attention

    • If so, where did this injury occur (mark all that apply: home, child care, some other place)

  • Flourishing

    • Comforts or helps others


Section 7: Middle Childhood and Adolescence (6 – 17 years)


Revisions:

  • Screen & media time (split into two separate questions)

    • Amount of time on an average weekday

    • Clarified “limiting the amount of time” and “monitoring the content”

      • reordered questions

      • will be emphasized in interviewer training


Deletions:

  • School performance in reading and language arts

  • School performance in math

  • Number of hours taking care of self in past week (SC 13+ years old)

  • Exercise to strengthen/tone muscles

  • Social competence scale

    • He/she shows respect for teachers and neighbors

    • Gets along well with other children

    • Is disobedient

    • Is stubborn, sullen, or irritable

    • Tries to understand other people’s feelings

    • Tries to resolve conflicts with classmates, family, or friends

  • Depressed mood

    • Feels worthless or inferior

    • Is withdrawn, and does not get involved with others

  • Flourishing

    • Able to relax and enjoy life

    • Religious or spiritual beliefs shape how SC lives life


Section 8: Family Functioning


Deletion:

  • Warmth and closeness of relationship



Section 9: Parental Health


Deletions:

  • Maternal exercise

  • Paternal exercise

  • Respondent exercise (if not SC’s mother or father)


Section 10: Neighborhood & Community Characteristics

Deletion:

  • Children in this neighborhood can grow up to be anything they want to be


Section 11: Additional Demographics

Deletions:

  • Length of time SC lived at current address

  • Past month - any day when respondent or anyone in family went hungry because they did not have enough money for food



Section 12: Additional Health Insurance Questions

(supported by the Office of the Assistant Secretary for Planning and Evaluation, DHHS)


Unchanged (relative to similar questions used for 2001 evaluation):

  • Main reason for being uninsured (K12Q01) – options modified to delete those unused in 2001 and add commonly reported “other reasons”

  • Length of current period of uninsurance (K12Q02)

  • Ever heard of Medicaid / CHIP (K12Q11, K12Q21)

  • Ever been enrolled in Medicaid / CHIP (K12Q12, K12Q22)

  • Ever applied for Medicaid / CHIP (K12Q15, K12Q25)

  • Date when last applied for Medicaid / CHIP (K12Q16, K12Q26)

  • Belief that child is currently eligible for Medicaid / CHIP (K12Q34)

  • Main reason child is not covered by ESI, if available to child (K12Q45, K12Q55, K12Q65)

  • Size of parents’ employers, if ESI is available to parent (K12Q46, K12Q47, K12Q56, K12Q57, K12Q66, K12Q67)


Revisions:

  • Main reason unable to enroll in Medicaid / CHIP (K12Q17, K12Q27) – was main reason application process was not finished, options modified to delete those unused in 2001 and add commonly reported “other reasons”

  • Perceived difficulty completing an initial application or re-enrolling (K12Q32, K12Q33) – did not previously include re-enrollment, asked only about “an application”


Deletions:

  • Secondary reasons for being uninsured

  • Source where first heard of Medicaid / CHIP

  • Secondary reasons for not wanting to enroll child

  • Ever tried to enroll but not finish application process
