Form ETA 678 is used to obtain information about student training for placement of students in jobs, further education or military service. It is used to evaluate overall program effectiveness and is the only form which documents a student's post-center status. The form is prepared by Job Corps centers and placement specialists for each student separating from Job Corps centers and has no further impact on the public.
