Scope of Instrument

This instrument collects:

• Personally identifiable information of individuals applying for an Ammonium Nitrate Registered User Number to include full name, address, e-mail, phone number, date of birth, place of birth, gender, citizenship, DHS Redress Number when applicable, passport or visa information when applicable, alien registration number when applicable, and photo identification information, IP addresses

• Any information necessary to verify an applicant’s enrollment in a DHS program with equivalent TSDB vetting

• Whether the individual applying for an AN Registered User Number is applying as an AN Seller, AN Purchaser, AN Facility Representative, and/or Designated AN Facility POC

• Whether the individual applying for an AN Registered User Number is a prospective AN Purchaser registering an agent or multiple agents

• Contact and location information about all facilities to which an individual registering as an AN Seller, AN Facility Representative or Designated AN Facility POC is assigned

• Updates and changes to personally identifiable information
