Scope of Instrument

This instrument collects relevant information necessary to process, conduct and resolve appeals or other filings, to include:

• Personally identifiable information of individuals who file a written Request for Materials requesting copies of the information on which denial or revocation was based

• Information deemed pertinent by, or upon which an appealing individual disputes the Department’s denial or revocation determination, as submitted by that individual in a Request for Appeal

• Other information necessary for processing the appeal
