In accordance with E.O. 12353, Public Law 102-393, and 5 CFR Part 950, required information is collected from 501(c)(3) non-profit charitable organizations that apply on an annual basis for participation in the Combined Federal Campaign. The information gathered is reviewed to determine eligibility in the program. OPM has made non-substantive changes to the 2011 CFC Applications: 2011 CFC National Independent Application (1647-A), 2011 CFC National Federation Application (1647-B), 2011 CFC Local Independent Application (1647-C), 2011 CFC Local Federation Application (1647-D), and 2011 CFC FSYA Application (1647-E). The revisions include date changes (e.g. 2010 to 2011), the removal of instructions related to an IRS Form that is no longer accepted, and, in the case of 1647-C only, an extra line to denote a charity's service address if it is different from its administrative office address. The changes do not include the addition or removal of any questions, and the burden remains unchanged. The revised forms are included in the Supplementary Document section.

The Office of Personnel Management (OPM) has centralized the process of verifying the 501(c)(3) tax-exempt status of applicant charities, which had been handled primarily by more than 200 local CFC offices throughout the United States, in response to a recommendation from the Government Accountability Office. Previous figures were provided based on estimates from those offices. Current figures are based on actual data processed by OPM.
