FCC Form 605 is a multi-purpose form used to apply for an authorization to operate radio stations and perform a variety of other miscellaneous tasks in the Ship, Aircraft, Amateur, Restricted and Commercial Radio Operators, and General Mobile Radio Services. The Commission is submitting this collection as an extension because we have re-arranged the lay out of the FCC Form 605; and clarified some of the instructions. These changes are not considered a revision and no new requirements have been added.

The Commission has reformatted the FCC 605 Schedule D to rearrange the layout of Question 2 for Vanity Call Sign Change; to further clarify the filing instructions; and to define the term "in-law".
