Abstract: Military members and their families are offered assistance in adapting to military life through the programs and services of the Airman and Family Readiness Center (A&FRC). These forms are used to collect demographic and personnel data, and record services provided to customers. These forms provide usage and trend data for program evaluation, targeting services, budgeting decisions as well as reports to DOD and Congress.
